Touch is a fast-moving and energetic sport. Players participate at their own risk and competition management accepts no responsibility for injuries sustained.
Claims for the Player Sport Benefit Fund will be accepted only if the following requirements have been fulfilled:
* The injured player has signed the Player Sign On Card & Player Insurance Form.
* The game referee has been advised of the injury and noted details on the score card.
* A claim form has been requested through the office within 60 days from the injury.
Non-Medicare Medical Expenses
The cover reimburses 80% of non-Medicare medical treatment and the maximum benefit for this section is $2000. All treatment must be certified by a registered medical practitioner and not subject to any Medicare rebate. All claims are subject to a $50 excess. Any expenses must be incurred within 12 months of the registered member sustaining the injury.
How to Claim?
(only if injury is reported to the game referee or venue organiser)
Step 1:
Obtain claim forms from City Touch by phoning 3275 2077 or emailing office@citytouch.com.au
Step 2:
Make sure you lodge your claim form within 60 days from the date of injury; otherwise your claim may be affected. Have the claim form fully completed and returned to the office.
Step 3:
Once treatment is finished post all original receipts to Po Box 8293, Woolloongabba, 4102 (within 12 months of the date of the injury)
